5. The applicant commits to immediately discontinuing sport driving activity and reporting any changes in their situation as specified in paragraphs 1-3 to the Motorsport driver’s Authority within 7 days.

6. The applicant commits to complying with all provisions of the Motorsport driver’s Law and its regulations, including insurance requirements, rules, and directives issued by the Motorsport driver’s Authority.

7. The applicant acknowledges the risks associated with competitive driving and pledges to act responsibly and with caution, with a focus on the safety of the audience and fellow competitors.

8. The applicant confirms that the motorsport driver’s license application has been filled out in accordance with all statutory requirements, including specific laws and regulations.

9. All information and details provided by the applicant in this declaration are complete, correct, true, and unaltered.

10. All documents attached to the online application form are true to the original, and their details are correct, complete, accurate, and unaltered.

11. The applicant agrees to update this declaration promptly if there is a material change in its contents.

12. The applicant commits to keeping the original documents in their possession for three years from the date of signing this document.

13. The applicant is aware that they must hand over the original documents immediately to the Motorsport driver’s Authority or any authorized party if requested by law.
